About Us

The Health Center at Standifer Place has evolved into a unique, comprehensive facility. From home care to compassionate hospice care, HCSP provides options for successive stages of life and changing levels of ability. Residents have convenient access to services on the journey from home care to long-term care, all on one campus.

Our services encompass a wide range of options to meet diverse needs, including Skilled Nursing, Independent Living, Assisted Living, and Adult Day Services. We also offer comprehensive Home Care Services and specialized Respiratory Services.

Beautiful Campus

Our dedicated team provides Alzheimer’s care in a secured environment, on-site professional medical services, fall and wound prevention programs, therapeutic recreation, and a dedicated respiratory therapy wing. Additionally, our rehabilitation department delivers extensive physical, occupational, and speech therapy to support recovery and well-being.

A multi-tiered staff of professionals provides comprehensive medical care. Licensed Physicians and Nurse Practitioners are available on a full-time basis. Registered nurses, assisted by LPN’s and CNA’s are scheduled 24/7. The nutritional services department customizes dietary care plans to address specific health issues and to aid in recovery.

Our Team

John R. Strawn

CEO

Mr. Strawn joined the Health Center at Standifer Place in 1999 and has more than 24 years of experience as an Administrator. He is a graduate of MTSU with a BBA in Business Administration.

Dave Boozer

ADMINISTRATOR

Mr. Boozer has been a licensed Nursing Home Administrator since 2007. Mr. Boozer earned a BA in Interdisciplinary Studies from Covenant College in 1990. He also earned his MBA from the University of Tennessee at Chattanooga in 2000.

Angela Breakey, RN

DIRECTOR OF NURSING

Mrs. Breakey graduated with a Bachelor’s degree in Nursing from The Mississippi University for Women Nursing experience, she is a certified Operating Room Nurse, School Nurse, Cardiac Care Nurse. She has lived abroad for several years with her military family this included England & Japan. She has been married for 28 years to Mike and has two boys Michael and Brice. She started working at The Health Center at Standifer Place in August of 2009 as the Staff Development Coordinator, and was later appointed to the Assistant Director of Nursing in 2015.

Tracy Carter

DIRECTOR OF SOCIAL SERVICES

Ms. Carter began working at Standifer Place in June 2013. She graduated from The University of Tennessee at Chattanooga in 2007 with a Bachelor of Science in Psychology and she earned a Masters of Education in Community Counseling in 2011 from The University of Tennesee at Chattanooga as well. Having started as a social worker, she was promoted to Director in April 2015.

Ken Stonebrook

ASSISTED LIVING ADMINISTRATOR

Mr. Stonebrook joined the Health Center at Standifer Place in 2009 and has been a licensed nursing home Administrator for over 15 years. He graduated from Southern Adventist University with a BBA in Marketing, and a BS in Long-term Care Administration. Ken runs the Home and Community Based Services and Assisted Living Facility for Standifer Place.

Grant Kunselman, BS, MBA

ASSISTANT ADMINISTRATOR

Mr. Kunselman is a licensed nursing home administrator in Tennessee. He earned a BS in Business Administration from Lee University and then an MBA from the University of Tennessee at Chattanooga. He has worked in the healthcare industry since 2012.
